PRESCRIPTION AND NON PRESCRIPTION DRUG INFORMATION
ATTENTION: PARENTS & GUARDIANS
Welcome back to school from Teri, the School Nurse!  With the start of school, there are a few things that I need to remind you of about our medication policy.
1.  If your student will be needing Tylenol (or any other over-the-counter) medication during school hours:
  •  It needs to be in the original container.
  • medication form will need to be filled out by a parent or guardian.
  • These are to be brought to the Nurse’s Office.
2.  If your student will be needing a prescription medication during school hours:
  •     This needs to be brought to the Nurse’s Office in its original pharmacy labeled container.
  •     A medication form will need to be filled out by a parent or guardian.
  •     These are to be brought to the Nurse’s Office.
3.  All students who are legally prescribed Ritalin (or any other controlled substance) absolutely cannot bring the medication to and from school.  An adult must deliver it to the school.

4.  All students need to have their Immunizations up to date.

Feel free to contact me with any questions that you may have.  Thank you so much for your cooperation.  I look forward to working with you throughout the school year.

Sincerely,

Teri Klosterman, School Nurse
